APPOINTMENT DECLARATION

I declare that the above information is true and correct and indemnify In Flow Kinesiology of any liability for any false or misleading statements given. 

It is also understood and accepted that the information provided by you, as well as all notes and information taken in every session is kept confidential, safe and secure and will remain the property of In Flow Kinesiology as part of client history records. Personal information may be used for notification of any future news, mini-retreats or services deemed as appropriate.

I further understand that 50% of the appointment is to be made upon the time of booking online and once I have confirmed my appointment with In Flow Kinesiology and / or Jacinta Gleeson I am subject to the cancellation policy.

Last minute cancellations with in 24 hours or no shows will be charged the full appointment fee.

I understand that all in-person Kinesiology sessions must be finalised on the day of appointment. I understand that In Flow Kinesiology accepts cash, direct bank transfer and card payments.

I hereby acknowledge that any advice given by Jacinta Gleeson of In Flow Kinesiology, in the written materials, any home reinforcements, self-help techniques and / or lifestyle recommendations etc, does not replace the advice of a medical practitioner.

I hereby give permission for Jacinta Gleeson from In Flow Kinesiology to facilitate and conduct a Kinesiology session on me.
